Carpal tunnel syndrome (CTS) is one of the most common disorders in neurology, affecting 3 to 5% of the adult population in the United States. Because of its high prevalence and potential for occupational disability, a variety of treatments for CTS have been proposed and studied since the 1940s.1 Surgical release was one of the first effective therapies and is supported by numerous controlled studies.2 Among the nonsurgical therapies, neutral position wrist splinting, as well as a brief course of oral prednisone, have also been supported by controlled trials.3,4 In 1957, Phalen and Kendrick reported improvement in 16 of 20 patients following local injection of corticosteroids5 and benefit, usually within 1 month of treatment, was confirmed in numerous studies over the next several decades, including a number of controlled trials.6,7 However, the majority of studies of both nonsurgical therapies and local steroid injection also documented high recurrence rates …
