In direct between-group comparisons, FTLD-Tau had greater pathology in left MFC than FTLD-TDP (p=0.029), while FTLD-TDP had more in left OFC than FTLD-Tau (p<0.001). Across the total cohort, left MFC pathology burden was associated with nonfluent speech (p=0.017) and left OFC burden with single-word comprehension impairment (p=0.045). Post-mortem density of pathology in GM was negatively associated with ante-mortem MRI CT (p=0.007), and, similar to digital pathology results, FTLD-Tau had lowest CT in MFC while FTLD-TDP had lowest CT in OFC.Conclusions: Microscopic pathology in PPA is lateralized to the left hemisphere with divergent anatomic distributions in FTLD-Tau and FTLD-TDP, and these are related to ante-mortem clinical features and MRI structural imaging.Study Supported by: NS044266, AG038490, AG015116, AG010124, P01NS053488, P01AG032953, P01AG017586, AG043503, NS088341, Penn Institute on Aging, Wyncote Foundation.Disclosure: Dr. Giannini has nothing to disclose.
