There are few circumstances in which the provision of experienced, specialized care is as critical as the determination of death by neurologic criteria (DNC) after catastrophic brain injury. The American Academy of Neurology Ethics, Law, and Humanities Committee noted that regarding DNC, “There may be nothing more injurious to the social contract between physicians and patients than inaccuracy in its determination.”1 Sadly, a recent survey of physicians from academic medical centers found that only 25% reported completing a DNC examination that is consistent with practice guidelines.2 A similar percentage reported completing DNC examinations despite lacking the training to do so. To put it kindly, we have an opportunity for improvement.
